Community Health Worker Program

Supporting Improved Health and well-being

Community Health Workers (CHWs) are trained public health workers who help keep community members connected with the health resources they need. Healthy Petaluma launched our CHW Program in 2021 using grant funding from Sonoma County. The program initially focused on lessening the disproportionate impact the COVID-19 pandemic had on underserved populations in southern Sonoma County. Our work has since expanded to address the needs of those at greater risk for poor health outcomes through health education, messaging, and resource navigation.

Your Trusted Messengers

Healthy Petaluma’s CHW’s work in the community providing blood pressure checks, offering COVID-19 supplies such as KN95 masks, rapid antigen testing kits, resource navigation lists, and free diapers and pull-ups. Healthy Petaluma is committed to deploying  the CHWs as trusted messengers in the community and working with health centers, nonprofits, and school systems to promote health, wellness, and resilience.
